The nominating authority gives the following assurances:
  1. All information supplied in this form is complete and correct;
  2. It is noted that SESAME will not be responsible for the payment of any costs or compensation arising from damage to or loss of personal property, or from illness, injury, disability or death of the trainee while he/she is travelling to and from or attending the training and it, the nominating authority, undertakes the responsibility for such coverage;
  3. The nominating authority confirms that all costs of travel, accommodation, subsistence allowance, insurance…etc relating to the training at SESAME, whether before, during or after this period will be covered from national or other sources and is aware that SESAME will not be responsible to cover or subsidize those costs;
  4. The trainee’s position in his/her institute of origin will be retained and he/she will receive during the training a salary and related emoluments to enable him/her to meet his/her financial commitments in his/her country;
  5. The selected trainee will conduct himself/herself in a manner compatible with his/her status as a participant in SESAME training and will refrain from engaging in any political and commercial activities.;
  6. No facts are known to the nominating authority regarding the reliability of the nominee which would obstruct giving him/her access to institutions where ionizing radiation is used.
